SOCIAL WORK CEU APPROVAL APPLICATIONS

Providers seeking CEU Approval through NASW-LA are asked to use the recently updated CEU Application form. The revised form includes a clickable link to an online portal to submit all applications and required attachments. Please note that payment is accepted by credit or debit card only. UPDATE: Our application form has been updated to include the credit card processing code. If you are using an older form, it may not have this information, and this could delay your approval. Please ensure that you’ve downloaded the most recent form here.

CEU Pre-Approval Add a Date Request/Change Request Form

• Applicants may request to add a training date or make changes to a previously approved application.
• Additional dates/locations or changes may be added within one (1) year of the date of the first workshop.
• The 12-month period begins with the first date of the first workshop as indicated in your original approval communication.
• There will be no revisions to this time frame.
